This document relies, in large part, on reviews of the evidence provided in comprehensive, high-quality evaluations of crime prevention programs and initiatives from Canada and from other countries. It is not intended to replace, but rather to accompany and complement, existing program resources. This document is based on findings from crime prevention literature reviews which identify model and promising crime prevention programs. It illustrates how to create successful prevention programs in an effective and sustainable manner with key actors and helps to understand how successful crime prevention can be implemented. It includes concise descriptions of innovative, promising and model prevention programs. It is primarily meant to be a source of inspiration for all those concerned with reducing delinquency, violence, and insecurity. This information is not intended to dictate what must be done nor is it intended to instruct organizations to replicate the models listed below. This document is part of an ongoing effort at the NCPC to promote and disseminate information and knowledge on effective crime prevention programs, strategies and initiatives. For this purpose, crime prevention practices should be integrated with the activities of existing community programs and services, build on the knowledge of risk and protective factors, and use evidence-based practices. It concentrates on two core activities: firstly, it supports targeted crime prevention practices, and secondly it builds and shares practical knowledge. Monthly Service Authorization Requests (SARs) are entered electronically in the Wraparound Milwaukee Synthesis system to authorize vendors in the Network to provide specific units of services to families. They also are responsible to help the family identify and obtain formal services through the Wraparound Milwaukee Provider Network to meet mental health, social and other support needs. Care Coordinators help develop the Care Plan to guide the delivery of services and treatment. Care Coordinators meet the child and family, conduct a strengths-based inventory, convene the Child & Family Team, and develop the care or treatment plan based on child/family needs, goals and formal and informal resources available or needed to support the family.įamily Needs and Safety Plan Help Identify ServicesĬare Coordinators utilize life domains to look at family needs and create safety plans for each child to ensure there are structured supports and supervision to ensure child and community safety. Wraparound Milwaukee employs Care Coordinators who are responsible for convening Child and Family Teams from which a wraparound plan of care is developed for each child and his/her family.
